How to Use the D&D Combat Tracker
Managing D&D combat gets complex fast — multiple combatants, changing HP, status conditions, and tracking whose turn it is. This D&D combat tracker handles all of it so you can focus on storytelling.
Step 1: Add All Combatants Before Initiative
Before rolling initiative, add every combatant — players, enemies, and allied NPCs. Enter their name, type, max HP, and AC. You'll enter initiative rolls next. Order added doesn't matter since the tracker sorts automatically.
Step 2: Enter Initiative Rolls
As each player rolls initiative (d20 + Dexterity modifier), enter the result in the initiative field. The tracker sorts everyone from highest to lowest automatically. On ties, players appear before enemies.
Step 3: Manage HP During Combat
When a combatant takes damage, use the red minus button to decrease their HP. Healing uses the green plus button. The HP bar turns red below 50% and flashes when a combatant reaches 0. Death saves appear for player characters at 0 HP.
Step 4: Track Conditions
Click the conditions button on any combatant to toggle status effects. All 15 standard 5e conditions are available. Active conditions show as colored badges under the combatant's name, making it easy to see who is poisoned, prone, or stunned at a glance.
Step 5: Advance Turns and Rounds
Click "Next Turn" to advance to the next combatant in initiative order. A blue highlight shows whose turn it is. When the last combatant acts, the round counter increments and combat returns to the top of initiative order.
Persistent Storage
Your encounter saves automatically between refreshes. This means you can close your laptop, return to the table, and continue exactly where you left off. Click "Reset Encounter" at the end of combat to clear all combatants for the next fight.

Is this D&D combat tracker free?
Yes, completely free with no account required. All data is stored locally in your browser — nothing is sent to any server.
Does the tracker save my encounter between sessions?
Yes. The current encounter is automatically saved to your browser's localStorage. Refreshing the page or closing the browser will restore your encounter exactly as you left it.
How do I track death saves?
When a player character's HP drops to 0, death save checkboxes appear below their entry. Track successes and failures with a click — at 3 successes the character stabilizes, at 3 failures they die.
Can I add monsters and players to the same encounter?
Yes. Each combatant has a type selector (Player or Enemy). This lets you track the full combat including monsters, NPCs, and player characters in one sorted initiative list.
How does initiative sorting work?
Enter each combatant's initiative roll when adding them. The tracker sorts all combatants highest initiative first. On ties, players go before enemies by default.
What conditions can I track?
All 15 standard D&D 5e conditions: blinded, charmed, deafened, exhaustion, frightened, grappled, incapacitated, invisible, paralyzed, petrified, poisoned, prone, restrained, stunned, and unconscious.
